 New in 2024, Doctor's signature is no longer required. Parent/Guardian Consent/Waiver is now required.

- Vikings Code of Conduct Contract


We will also need:

- Copy of 2024-2025 Report Card with all 4 Quarters

- Original Birth Certificate
