Season Starts August 1st!

Here are the 5 important documents that must be turned in:

1. Physical - MUST be on 2022 Pop Warner Physical Form (Note: we will have two doctors available the first week of practice onsite for physicals for a reasonable cost).

2. Original Birth Certificate (This will be returned to you after Certification Day)

3. Copy of 2021/2022 year-end report card with all 4 quarters (or homeschool form)

4. 2022 Pop Warner Player/Participant Contract

5. Vikings Code of Conduct Form

All forms are available under the Folders section of the BAND App or contact your team manager. See you Monday the 1st for our first practice!
